Rely Medical, LLC, ("we", "our" or "us") provide this website and services expressly subject to the following terms and conditions ("Terms"). We may change or modify these Terms from time to time. If a modification or change is unacceptable to you, you may terminate this agreement by ceasing to use the website. Please review these Terms to endure they are acceptable to you. While it is very unlikely to occur, Rely Medical, LLC, is not responsible for any damage to data, software, computer, telecommunications or other equipment (including damage caused by virus transmission) that you may experience as a result of visiting this website or any of its links.

Your account

If you use this site, you are responsible for maintaining the confidentiality of your account and password and for restricting access to your computer, and you agree to accept responsibility for all activities that occur under your account or password. Rely Medical, LLC, reserves the right to refuse service, terminate accounts, remove or edit content, or cancel orders in their sole discretion.

Automatic Renewal

If you are placing an order online or by telephone as part of our automatic renewal program (Rely VIP Autoship Program), your membership in the program will remain in effect until it is cancelled. We may, in our sole discretion, terminate your membership in the program at any time without notice to you.

IF YOU ARE A MEMBER OF THE RELY VIP AUTOSHIP PROGRAM AND YOU HAVE PROVIDED US WITH A VALID CREDIT CARD NUMBER OR AN ALTERNATE PAYMENT METHOD, EACH SHIPMENT WILL BE AUTOMATICALLY PROCESSED AT THE TIME OF EACH SUCCESSIVE SHIPMENT AND/OR INSTALLMENT AND WILL BE BILLED TO THE PAYMENT METHOD YOU PROVIDED TO US AT THE TIME OF YOUR INITIAL PURCHASE AND ENROLLMENT. IF YOU WISH TO CANCEL YOUR PARTICIPATION IN THE RELY VIP AUTOSHIP PROGRAM, YOU MAY DO SO BY CALLING A CUSTOMER SERVICE REPRESENTATIVE AT 888-529-2308.

You are obligated to provide current, complete, and accurate information for your billing account. You are responsible for promptly updating all information to keep your billing account current, complete, and accurate (e.g., change in billing address, credit card number, or credit card expiration date). You must promptly notify us if your credit card information is cancelled or is no longer valid (e.g., loss or theft). Changes to such information can be made by calling 888-529-2308. If you fail to timely update your credit card information or fail to timely pay by check, your membership in our automatic renewal program may be terminated and your account may be sent for collection.

Return/Refund Policy

You may return items within 10 days of receipt and in unopened condition, in accordance with the Return Policy published on this website, if you are uncertain about your right to return the product, you may also call a customer service representative at 888-529-2308 for assistance.

Reviews, comments, communications, and other content
